Forms

The paperwork you need for training—easy to find, easy to finish.

Nobody signs up for scuba because they love paperwork. But a few key forms keep your training on track—and this page is where we keep the most commonly requested documents in one place.

If you’re already enrolled, use this page to download, complete, and return whatever your course requires. If you’re thinking about enrolling, this is also a good preview of what you’ll be asked to fill out so there are no surprises later.

What you’ll typically use this page for

You’ll usually come here when a training page or an email mentions a required document—often a medical statement, acknowledgments, or other course paperwork.

Our goal is simple: help you get the right form, fill it out correctly the first time, and avoid last‑minute delays.

Forms library

We keep this section reserved for the actual downloadable forms.

If you don’t see the form you’re looking for, it may be:

  • Included directly in your course materials
  • Specific to your course start date or training track
  • Something we send to you after registration

When in doubt, reach out and we’ll point you to the correct document.

Common issues to avoid

How to complete forms without headaches

A few tips that save time (and back-and-forth):

Read every line before you start. Most delays happen when a signature, date, or required field gets skipped.

Use your legal name and consistent contact info. If your registration uses one name and your forms use another, it can slow things down when we’re matching paperwork to a student.

Don’t guess on health-related questions. If a form asks for medical information and you’re unsure how to answer, pause and get clarity from the appropriate professional. It’s always better to confirm than to cross your fingers.

Returning your forms

Different courses and documents may have different return instructions. If your form doesn’t clearly say where to send it, use the contact information on our site and tell us:

  • Which course you’re in (or planning to take)
  • The name of the form you’re working on
  • Whether you’re stuck on a specific question or just need the right place to submit it

We’ll get you moving in the right direction.

Not sure what you need?

If a training page referenced “forms” and you’re not sure which one applies to you, contact us with your course name and what you’re trying to schedule next. We’ll help you identify the exact document(s) you need so you can keep your momentum—without turning paperwork into a project.
